The FRO New Member Intake Form is intended to help us organize the process of bringing new people into FRO. Please fill out the form and return it to FRO by one of three ways:

• Quickest: Email the form with your responses to fro@riseup.net
• OR Mail the form to FRO, P.O. Box 6024, Olympia, WA 98507
• OR Drop the form for us at our drop box at Last Word Books, 211 4th Ave. You may also pick up a form there. Please ask the person at the counter to put the completed form in our drop box.

Your form will be reviewed by the FRO collective. Please be patient with us and send us a follow up email if you do not hear from us in a couple weeks to assure we received and are reviewing your form. Your invitation will be contingent upon our consensus process. One of our volunteers will notify you via the contact information you provide on the form on next steps.

We highly encourage and welcome other radicals, folks with a passion for direct action broadcasting, and the like to join. We especially encourage folks interested in indie media, sharing conscientious music, reporting on community events and more to be part of our longstanding collective.
